We’re delighted you’re going to be a model for one of our Inspired Cosmetic Training courses.

Please read the information below carefully, all models, provided they are happy with the information need to sign this form to agree to our terms and conditions.

Model Information

Please be aware that you are a model for a training course and will be receiving treatment in front of other delegates (Nurses, Dentists, Doctors) who may ask questions and be involved in the assessment and treatment plan. It is therefore imperative that mobile phones are switched off or on silent while they are in the clinical room.

Please Read Relevant Parts – A or B depending on the course you are a model for:

A. Foundation Botulinum Toxin & Filler Model Treatments:

During assessment and consultation we will discuss suitable treatments options for you. As the model you should be suitable for both Botulinum toxin type A and dermal fillers. Please note that the delegate will not pass the course if they do not have the opportunity to inject both products. The tutor will prescribe the Botulinum type A as this is a prescription medication. Please be aware that this is a foundation level training course and that we will not be discussing advanced treatment options. You should be suitable for the following treatment:


Botulinum Type A

Helps to minimise the appearance of fine lines and wrinkles in the following areas: Glabellar (frown), Frontalis (forehead), and Peri-orbital (crows feet/sides of eyes).

Dermal Fillers

Designed to effectively reduce the appearance of unwanted fine lines and wrinkles, contour and create volume, and to revitalise the skin. We will be working on defining and volumise lips on this course.

B. Advanced Level Model Treatments:

For our advanced courses we’re going to work on the following treatments. Inspired Cosmetic Training or the delegate will have discussed with you the areas we’ll be working on.

Advanced Fillers: Lips, cheeks, jawline, nasolabial folds, marionette lines, chin, full face, lip dissolving

Advanced Botox: Masseters, gummy smile, dimpled chin, smokers lines, bunny lines, downturned mouth, Nefertiti lift, hyperhidrosis, platysmal bands

Skin Boosters & Polynucleotides: Face and body

Skin Peels: Face

Microneedling: Face

Is there anyone you can’t ask to be a model?

  • You must be 18 years or over, be in good health, not pregnant or breastfeeding and be suitable for treatment with mild to moderate fine lines, wrinkles and/or dermal fillers (depending on course attending).
  • For Botulinum Type A, if you have hypersensitivity to botulinum toxin or any of it’s constituents we need to exclude you. Anyone with an infection at the proposed injection site, neuromuscular disorders such as myasthenia gravis, certain antibiotics and anticoagulants.
  • For dermal fillers, exclude anyone from treatment who has a known hypersensitivity to hyaluronic acid or lidocaine. Anyone with an infection at the proposed injection site, autoimmune diseases, bruising or bleeding disorders, or using anticoagulants and immunosuppressant drugs will also be excluded.
  • Ideally models should not be needle phobic and preferably someone who has had injectables, tattoos or piercings as they are usually less nervous.
  • Skin Boosters as above

*Models should not have had Botulinum Type A within the last 3 months or dermal fillers in the last 6 months.

How long will you be needed?

We do our best to ensure the course runs to schedule. However, due to the nature of this training course, you may be waiting for a significant time.

*Models must not wear any make up as it will be removed prior to treatment*

Inspired Cosmetic Training: Model Terms & Conditions

Model should complete this form ASAP or at least 5 days before course date.
 
1. You are required to complete a full medical history in advance of attending the course, we will not be held responsible for any omission or incorrect information given. You will also have to sign and return the patient information leaflets for Botulinum Type A and Dermal Fillers (Foundation course). For Advanced models, read relevant patient information leaflet.
 
2. For all Foundation courses, models will be required to consent to BOTH Botulinum Type A and Dermal Fillers, as our delegates need to work on both Botulinum Type A and Dermal Fillers to pass their course.
 
3 a/. I understand the treatments that will be worked on for the Foundation course are:
 
  • Botulinum Type A: crows feet, forehead and frown lines
  • Dermal Lip Fillers: Lips
 
If you do not wish Botulinum Type A, Dermal Fillers or any of these treatments, unfortunately we cannot use you as a model for the Foundation course.
 

3 b/. I have been informed of the treatment(s) that will be worked on at Advanced level, which may include (depending on course attending):

  • Advanced BOTULINUM TYPE A (bunny lines, gummy smile, dimpled chin, downturned mouth, neck bands, hyperhidrosis, masseters)
  • Advanced Fillers (lips, chin, jaw, nasolabial folds, marionette lines, cheeks)
  • Skin Booster / Polynucleotide treatment
  • Skin Peel treatment
  • Microneedling treatment
 

4. You will have to sign a consent form on the day of the course. It is important you read the consent form carefully as it will detail the risks associated with Botulinum Type A and/or Dermal Filler treatment and potential complications.

Please note: Risks and complications occur infrequently, should complications occur, and any additional consultations and treatment be required this will be arranged with the tutor and Inspired Cosmetic Training. You will incur a charge for any follow up consultations and treatments your model may require.

 
The practice of medicine and surgery is not an exact science. Although good results are expected, there is no guarantee or warranty expressed or implied, on the results that may be obtained.
 
5. These terms and conditions ensure that you are booked as a model on the day and to ensure the smooth running of the course. If a model does not attend the course, it can affect the amount of practical experience our delegates receive.
 
6. We will store all your personal data in line with current GDPR privacy policies and will only share your information will the other relevant medical companies we work with. See link attached.

If you are attending as a model arranged by Inspired Cosmetic Training, you do not need to provide delegate details. Leave the delegate name and email fields blank.
