Consent to Medical Photography - Course Model

Patient Information Leaflet

Inspired Cosmetic Training has adopted a Policy for photography and video recording of patients. This policy follows the Data Protection Act 2018 and the General Medical Council’s guidance on ‘Making and using visual and audio recordings of patients 2011’. 

This Consent form allows you (the model) to control what your images are used for. As a minimum standard, photographs will be retained as part of your medical record so that you and your doctor or nurse can look back and use them for comparison. In line with the General Data Protection Regulations you have the right, at any point in time, to access and control all documentation held by the company relating to your treatment with us.

Form E - Consent to Medical Photography

If you are attending as a model arranged by Inspired Cosmetic Training, you do not need to provide delegate details. Leave the delegate name and email fields blank. 

If a delegate has arranged you as their model, please complete the delegate name and email below.

Photo Marketing Consent

I consent to any photographs/videos being shown on public platforms such as the Inspired Cosmetic Training website, Twitter, Instagram and Facebook sites, publications.

As a model you have to submit photos to be approved within 3 working days.

 

Foundation Course Models

Please upload the following photos:

  • Front profile relaxed
  • Front profile - Eyebrows raised as much as possible
  • Front profile - Eyebrows furrowed as much as possible
  • Side profile - Scrunching eyes
  • Lips
